Setting eligible endometrial cancer patients on an innovative course

pMMR endometrial cancer is a complex and highly heterogeneous disease, associated with a poor prognosis, which accounts for 70-80% of all advanced or recurrent endometrial cancer patients1-3

Only about 1 in 5 advanced/recurrent EC patients survive beyond 5 years5,6

Multiple histologies and overlapping molecular subtypes of aEC respond differently to treatment1,7,8,15

IO regimen outcomes seen in dMMR aEC have not been replicated in pMMR patients4,9,10,20

Patients with advanced/recurrent pMMR EC need more effective and durable treatment options.1,3,4

pMMR tumors are heterogeneous, with diverse genomic drivers. Compared to dMMR EC tumours, they have a lower tumour mutational burden, scarce immune infiltration and therefore limited ability to mount an antitumour response.14-16

 

Immunotherapy + platinum chemotherapy drives direct tumour cell killing. However, blockade of PD-L1 alone provides limited immune response because of the reduced mutational burden and neoantigen load of pMMR tumours1,14,15,17-19

 

PARP inhibition induces DNA damage and cell death which may enhance immune priming and stimulate robust  antitumour immune responses.11-13

Indication
  • IMFINZI in combination with carboplatin and paclitaxel is indicated for the first-line treatment of adults with primary advanced or recurrent endometrial cancer who are candidates for systemic therapy, followed by treatment with:
    • IMFINZI as monotherapy in endometrial cancer that is mismatch repair deficient (dMMR)
    • IMFINZI in combination with olaparib in endometrial cancer that is mismatch repair proficient (pMMR).
  • Lynparza in combination with durvalumab is indicated for the maintenance treatment of adult patients with primary advanced or recurrent endometrial cancer that is mismatch repair proficient (pMMR) whose disease has not progressed on first-line treatment with durvalumab in combination with carboplatin and paclitaxel.
