Why luteinizing hormone releasing hormone (LHRH) agonists?

Hormone therapies treat prostate cancer by blocking the production or use of androgens1-6

Androgens are essential to prostate function and promote the growth of both normal and cancerous prostate cells by binding to and activating the androgen receptor.

 

Reducing androgen production by the testis is a therapeutic approach to blocking the production or use of androgen, know as androgen deprivation therapy (ADT). This includes:

 

  • Orchidectomy LHRH agonists
  • LHRH agonists e.g. ZOLADEX (goserelin)
  • LHRH antagonists
  • Oestrogens

LHRH agonists decrease serum testosterone causing prostate tumour regression1

ZOLADEX, is a synthetic analogue of naturally occurring LHRH. On chronic administration, ZOLADEX results in inhibition of pituitary LH secretion leading to a fall in serum testosterone concentrations in males

 

  • Initially, ZOLADEX, like other LHRH agonists, may transiently increase serum testosterone concentration in men
  • ~21 days after the first depot injection, testosterone concentrations in men have fallen to within the castrate range and remain suppressed with continuous treatment:
    • every 28 days, when a 3.6 mg Zoladex depot is given to the patient
    • every 12 weeks, when a 10.8 mg Zoladex depot is given to the patient

Why Zoladex?

A 2018 systematic review of 16 studies on the clinical differences and treatment of LHRH agonists goserelin, triptorelin and leuprorelin for prostate cancer concluded:

In the absence of robust comparative survival and toxicity data, direct comparisons between products should not be made. Consider a LHRH agonist supported by clinical evidence and the longest follow up in randomised phase 3 trials in high-risk localised and locally advanced

prostate cancer.8-11
